| COBRA Administration
The following is an overview of the COBRA process that occurs upon termination of employment or the date an individual is no longer eligible for coverage.
RESPONSIBILITIES INCLUDE:
Initial Notice:
- Employer terminates COBRA Eligible Individual’s coverage with health and/or dental carrier.
- Employer completes EBA COBRA notification form and forwards to EBA.
- EBA will send notice, via certified mail, to employee and/or dependent notifying the individual of their COBRA rights and the monthly cost to continue coverage.
- If the individual responds that they choose to elect coverage, EBA will send a premium remittance booklet to the COBRA Eligible Individual for remittance of COBRA payments
- When the individual makes payment retroactive to termination date, EBA notifies the Employer to reinstate coverage with health and/or dental carrier. COBRA payments are made payable to EBA. (via email with read receipt)
- EBA will deposit COBRA payments, retain the 2% administrative surcharge and then remit applicable monthly COBRA premiums to the Employer.
- EBA will monitor COBRA payments to ensure timely payments. The Employer will be notified, by EBA, when to terminate a COBRA participant for failure to pay or exhaustion of the COBRA period.
- EBA will provide monthly status reports of COBRA participation to the Employer.

NOTE: EBA always recommends terminating the employee immediately from your health insurance plan. It is their responsibility to get the payments into EBA to ensure their cards will be turned back on.
